Holistic Needs Assessment (HNA)

The HNA and care plan allows a cancer patient to identify issues that they are concerned about; these could be physical, social, practical or spiritual.  Holistic simply means that the assessment looks at how cancer has affected the whole person.  Macmillan hopes that this will ensure that services are targeted where they are needed most and make sure all your needs are met.

Can I have an assessment?

Everyone who has a diagnosis of cancer is entitled to an assessment.  You can ask for an assessment at any stage of your cancer journey and it can be repeated as often as you feel is appropriate.  Initially the Clinical Nurse Specialist may complete the assessment during a clinic appointment; however you can request further assessments via the Macmillan Centre with one of our Community Navigators.

How long will it take?

An assessment can take up to about one hour, but if you need longer we can always arrange a follow-on appointment.

Where would it take place?

The first assessment may be done by a Clinical Nurse Specialist in a clinic but any subsequent assessments can take place in the Macmillan Information Centre.

Who will see it?

The Community Navigator completing the assessment will ask your permission to share the care plan with your GP and Clinical Nurse Specialist.
